Glucose Guards is a proprietary software and program that was developed to support the decrease in A1C levels, promote and sustain healthy behaviors, and support physicians with rigorous payor requirements. As a result of the Glucose Guards approach, providers have been successful in meeting quality measures and shared savings specific to this patient population.

Glucose Guards offers a cellular-enabled glucometer and platform that facilitates Remote Patient Monitoring (RPM), Medical Nutrition Therapy (MNT), and Diabetes Self-Management Training (DSMT), which are reimbursable services for the providers. Most recently, expanded services include patient support for hypertension and weight management; and currently, we are developing a support platform for Continuous Glucose Monitoring (CGM).
